Adult Membership (Package Value $1012.00)

• Investment: $70/month + $199 Lifetime Activation Fee
• Who It’s For: Patients 13+ with regular cleanings and absence of periodontal disease

You Save $175 per year!

• Professional dental cleanings (2 per year)
• Doctor exams (2 per year)
• Needed x-rays (as determined by your doctor)
• Emergency exam (if needed)
• Fluoride treatments, up to 2 per year
• 10% off all other treatments (some exclusions may apply)
• Activation fee includes first month of membership

Perio Membership (Package Value $1565)

• Investment: $95/month + $199 Lifetime Activation Fee
• Who It’s For: Patients enrolled in active periodontal maintenance due to prior treatment of gum disease

• Perio maintenance cleanings (up to 4 per year)
• Doctor exams (2 per year)
• Needed x-rays (as determined by your doctor)

• Investment: $55/month + $199 Lifetime Activation Fee

Legal and Disclaimers: Loyalty clubs are not insurance, but a payment arrangement provided by Beverly Hills Aesthetic Dentistry ™, Dental Group of Jamileynn M. Hanam-Jahr, DDS, PC for services rendered. Loyalty Club Memberships are provided exclusively to uninsured patients of our practice and shall not be considered pre-payment for future services or payment for access to discounted services. Rather, our patient members are electing to make regular monthly subscription payments for preventive care instead of paying at the time of each service. Payment for any cosmetic or restorative treatment is due at the time of service. Club dues must be current in order to receive services. A lapse of 28 days in dues will require a reactivation fee of six moths’ dues. Loyalty club discounts may not be combined with any other offers or discounts. It is solely the patient’s responsibility to schedule and keep their appointments. No refunds will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments. A patient member’s monthly agreement is with the legal entity of Beverly Hills Aesthetic Dentistry ™, Dental Group of Jamielynn M. Hanam-Jahr, DDS, PC. Monthly dues and fees for dental services may change at any time.
